Our records indicate that you did not have a social security number or other taxpayer identification number when you opened your HUECU account. For that reason, you completed an Internal Revenue Service Form W-8BEN.

If in the interim, your tax status has changed to resident alien (permanent resident) or became a US citizen since account opening, please select and complete the Internal Revenue Service Form W-9 option provided.

Federal regulations require that we periodically update this information. We request that you complete and electronically sign the appropriate form as soon as possible to avoid receiving this reoccurring notifications:

Please complete and electronically sign the applicable form above and return it following the instructions provided.

Also, please note, that if you are a non-resident alien and have obtained a social security number or other taxpayer identification number, please enter that number on line 5 while completing the Form W-8BEN.

Failure to update and sign this form may result in the Credit Union having to backup and withhold a percentage of interest earned as required by federal regulations.
